Patient Responsibility Agreement

1. I am at least 18 years of age and an adult of sound mind and judgement.

2. I am permitted by the laws in my residing country to receive a medical test kit, undergo clinical testing, receive clinical services, and/or the recommended treatment or medication, and that I am requesting these for my personal use. I enter into this Agreement in compliance with the laws of my country.

3. I am the intended patient and am not intentionally ordering prescription medication or test kits on behalf of somebody else.

4. I have full capacity and understanding to complete a consultation and am doing so of my own free will. I am not under duress or being coerced into placing an order on this website.

5. I, the patient, have had a recent examination with my GP (General Practitioner) or a local doctor who has evaluated my current clinical picture and past medical history and considered it to be at a satisfactory level. I agree and understand that the purpose of this service is to support, not replace, the relationship with my healthcare providers, local Doctor or GP.

6. I fully understand that the service euroClinix provides does not replace my GP or healthcare provider.

7. I am responsible for providing accurate, up to date and complete information about my medical history, including any pre-existing conditions, allergies, medications, and previous hospitalisations or surgeries. I understand the importance and the reasons for, and therefore agree, to fully disclose the list of medications that I am currently taking.

8. I have reviewed, assessed, and fully understand, as well as having the opportunity to discuss with a healthcare professional, all the risks, benefits and common side effects of the prescription treatment I am requesting. I have also reviewed the online materials provided on the euroClinix website, including various articles, blogs, literature and informative web pages offering detailed explanations on the condition, possible medications, and treatment options.

9. I will promptly inform my healthcare provider if there are any changes in my personal information, such as my address, phone number, or insurance coverage.

10. I understand that euroClinix may use secure electronic communication methods to communicate with me regarding my healthcare, and I am responsible for regularly checking and responding to such messages.

11. I will actively participate in my healthcare by asking questions, seeking clarification, and following the treatment plan recommended by my healthcare provider.

12. I understand that euroClinix has the right to terminate their professional services if I fail to comply with any of the terms outlined in this agreement.

13. I understand that complete transparency is absolutely necessary for my personal safety and I will uphold complete transparency at all times.

14. I agree to immediately contact my Doctor for any necessary check-up, care or intervention in the event that I should experience any side effects or complications or have any questions in respect of the medication(s). The prescribing clinician and the dispensing pharmacy may also be contacted if the need arises. I accept that the prescribing clinician or an appointed representative may contact me for any reason whatsoever even if I have not requested them to do so.

15. I have disclosed in full, all information concerning my health and medical history that may be relevant to my request for treatment, medication or test kits.

16. I have not omitted or misrepresented any information or statement of fact during the consultation process.

17. I will examine the goods as soon as possible after receipt and notify you of any fault, damage or incompleteness within 3 days from the time of delivery.

18. By using this website and services, and undertaking an online medical consultation and purchasing treatment, medications or test kits, I acknowledge that I have read and understood the terms of this Patient Responsibility Agreement and the Terms and Conditions of Business, and I agree, to abide by, and be bound by those terms in these Agreements.
