What is Sumatriptan (Imigran)?

Sumatriptan, often sold under the brand name Imigran, is a medication used to treat migraine and cluster headaches. It reduces symptoms of migraine headaches and can help reduce other migraine symptoms, such as nausea, vomiting, and sensitivity to light and sound.

It is not a preventative treatment and should be used for acute migraine attacks.

How does Sumatriptan (Imigran) work?

The exact cause of migraine attacks is unknown, but it is thought they are caused by a temporary widening of blood vessels in the head. A process which is influenced by a chemical in the brain called ‘serotonin’.

Sumatriptan is a type of drug called a ‘triptan’, or a serotonin receptor agonist (5-HT1 agonist). It affects the serotonin receptors, which causes your brain’s blood vessels to constrict and alleviates symptoms.

As well as Sumatriptan, there are several triptans available, including Almotriptan (Almirall), Zolmitriptan (Zomig), Rizatriptan and Naratriptan. These are available through several different forms including tablets, nasal sprays and some even as subcutaneous injections.

How do I take Sumatriptan (Imigran)?

Take this medication exactly as your doctor has directed. For full instructions, you should refer to the patient information leaflet.

Tablets

Sumatriptan tablets are available in two different doses: 50 and 100mg tablets.

You should swallow one tablet with water as soon as you feel a migraine attack coming on.

The medication should work within thirty minutes to an hour. If the first tablet has no effect, do not take another tablet for the same attack. If a migraine improves but then returns after at least two hours after the first dose, you can take a second dose.

Over-the-counter painkillers, such as ibuprofen, aspirin, and paracetamol, are safe to use alongside Sumatriptan unless your doctor has advised against this.

You should not exceed a daily dose of 300mg within a 24 hour period. If you take more than prescribed, please seek immediate medical attention even if you feel well.

Nasal spray

The Imigran nasal spray is available in a 20mg dose.

One spray (one dose) should be administered to one nostril. You should not spray twice nor should you spray into both nostrils. If you take too much, you should seek immediate medical attention.

A full description with diagrams can be found in the patient information leaflet, but a brief description on how to use the medication is as follows:

  1. Remove the nasal spray from the blister packaging just before you intend to use it
  2. Get into a comfortable position, either sitting or standing
  3. Blow your nose if you feel like it’s blocked or you have a cold
  4. Hold the finger grip on the nasal spray gently with your fingers and thumb, do not press the plunger yet
  5. Block one nostril by pressing a finger firmly on the side of our nose, doesn’t matter which nostril you choose
  6. Then put the nozzle of the nasal spray into the other nostril as far as it feels comfortable, about 1cm in
  7. Start to breathe in gently through your nose and as you breathe in, press the blue plunger firmly with your thumb
  8. Remove the nozzle and your other finger from your nose then keep your head upright for 10-20 seconds. Make sure to breathe gently into your nose and out through your mouth to make sure the medication stays in your nose.
  9. Safely and hygienically discard the empty nasal spray

Your nose may feel wet and you may experience a strange taste in your mouth after administering the spray. This is normal, but will quickly go away.

What are the side effects of Sumatriptan (Imigran)?

As with any prescription medication, Sumatriptan can cause some side effects. However, the most common side effects are mild and go away quickly on their own.

Some of the most common side effects include:

Common:
  • Dizziness
  • Tiredness
  • Sensory disturbances
  • Flushing
  • Shortness of breath
  • Muscle pain
  • Nausea or vomiting
  • A feeling of heaviness or pressure
  • Fatigue
  • Hot or cold sensations

What precautions should I take?

It is important that you are prescribed this medication from your doctor before ordering online so that they can assess your reaction to it and determine if it is right for you.

This treatment is available online from euroClinix if you are over 18 years of age. It is not recommended for those over the age of 65. However, if you have any preexisting conditions, you should notify your prescriber.

If you are pregnant or breastfeeding, you should ask your doctor before using Sumatriptan as traces of Sumatriptan can be found in breast milk. Whilst not known to be harmful, a doctor may be able to suggest an alternative medication if they feel it’s appropriate.

Some other contraindications include:

  • had a heart attack or stroke (including a ‘mini-stroke’ known as a transient ischaemic attack)
  • circulation problems in the legs (peripheral vascular disease or thrombosis)
  • heart disease
  • high blood pressure (uncontrolled)
  • reduced liver function (hepatic impairment)
  • an allergy to sumatriptan succinate, succinate salts or any of the other ingredients listed

You should always inform your doctor of any medication or supplement you are taking, even if it is not prescribed, as some medications may interfere with Sumatriptan, and vice versa.

This includes:

  • MAOI inhibitors (monoamine oxidase inhibitors) - to treat depression
  • SSRIs (selective serotonin reuptake inhibitors) and SNRIs (serotonin and norepinephrine reuptake inhibitors) - to treat anxiety and depression
  • St. John’s Wort - herbal medicine
  • other migraine medication (e.g ergotamine) or other triptans

Combining antidepressants with triptans can result in serotonin syndrome which causes insomnia, restlessness, confusion among other symptoms. It’s not a fatal condition, but can cause temporary discomfort.

Medication Overuse Headache (MOH)

Rebound headaches, or MOH, are the result of taking triptans very regularly. This is when triptans start to cause headaches, rather than treat them. If you take triptans for more than 10 days each month, you may be more at risk of developing rebound headaches and may benefit from specialist treatment.
