A doctor’s guide to the highest dose of Wegovy

The 7.2 mg dose of Wegovy is the highest strength available, and offers the potential for significant weight loss.

However, it is only available to some patients, and there are some important things to consider before increasing your dose from 2.4 mg. In this article, I outline key information about the 7.2 mg dose of Wegovy so you can see if it’s right for you.

What is Wegovy 7.2 mg?

Wegovy is a weight loss treatment that contains the active ingredient semaglutide, and 7.2 mg is the highest dose available. It works by suppressing your appetite and making you feel fuller after eating.

It is 3x stronger than the next dose down, and offers more effective results to those who have not achieved significant weight loss on the 2.4 mg dose.

When is Wegovy 7.2 mg prescribed?

The 7.2 mg dose of Wegovy is prescribed to people who haven’t seen significant weight loss on 2.4 mg. This offers another opportunity to reach your weight loss goals even if Wegovy hasn’t worked for you yet.

However, Wegovy 7.2 mg cannot be prescribed to everyone. To be prescribed the 7.2 mg dose, you must:

  • Have a BMI of 30 or over, 27.5 or above for certain ethnic groups, or 27 and above with a weight-related condition
  • Have been taking the 2.4 mg dose for at least 4 weeks
  • Experience no significant side effects on 2.4 mg

This ensures you can handle the lower dose before increasing it further and reduces the risk of side effects.

Why some people may need to use 7.2 mg Wegovy

For most of my patients, the 2.4 mg dose of Wegovy is enough to achieve significant weight loss over time. Others may need a little extra to reach their goals. This is where the 7.2 mg dose comes in.

Here are some reasons you may not be seeing significant results on 2.4 mg Wegovy:

  • You’ve hit a plateau: if you saw initial success but the scale hasn’t moved for a while, you may be experiencing a weight loss plateau. This happens when your body gets used to Wegovy at the current dose.
  • 2.4 mg isn’t strong enough: everybody responds differently to treatment. If you never noticed much of a change, you may simply need a higher dose to see progress.
  • You have an underlying health condition: medical conditions like hyperthyroidism (an overactive thyroid) and PCOS (polycystic ovary syndrome) can make weight loss far more difficult, even on Wegovy.

How effective is Wegovy 7.2 mg?

The 7.2 mg dose of Wegovy can offer significantly better weight loss results than the 2.4 mg dose.

Here’s what was found in a trial on Wegovy’s effectiveness:

  • Patients taking 7.2 mg lost an average of 21% of their body weight after 72 weeks
  • Those taking 2.4 mg lost an average of 18%
  • 33% of people taking 7.2 mg lost more than 25% of their body weight

Side effects of Wegovy 7.2 mg

There are no unique side effects of the 7.2 mg dose of Wegovy, but the risk of experiencing them is higher compared to the lower doses.

Here are the most common side effects of Wegovy, and the percentage of patients likely to experience them, according to the STEP UP trial:

Nausea 38.9%
Diarrhoea 24.2%
Vomiting 22.1%
Dysaesthesia (unusual or unpleasant sensations) 21.6%
Constipation 20.4%
Hair loss 5.3%

These results are from patients who tolerated the 2.4 mg dose well. This shows an increased likelihood of developing side effects despite not experiencing any on lower doses.

Keep in mind, however, that side effects usually go away within weeks as your body gets used to the medicine.

Is Wegovy 7.2 mg available in UK?

As of 15 July 2026, the 7.2 mg Wegovy pen was approved in the EU for weight loss. This offers new possibilities for weight loss in patients who have not responded well to lower doses of Wegovy.
